Measurement of Blood Loss in Cardiac Surgery: Still Too Much
2018, Annals of Thoracic SurgeryInitial hematocrit predicts the use of blood transfusion in the pediatric trauma patient
2014, Journal of Pediatric SurgeryCitation Excerpt :Conventional teaching holds that Hct remains stable for several hours after trauma because “patients bleed whole blood” and compensatory mechanisms for fluid shifts have not yet taken place [3,5,7]. However, there are multiple controlled studies in animals [8–11] and humans [12–15] showing an almost immediate drop in Hct following hemorrhage. As the initial blood draw at our trauma center is typically performed within 5 minutes upon arrival, sent to a laboratory with results usually available within 15 minutes, our study's findings support the conclusion that the admission Hct implies that fluid shifts from the interstitial space into the vasculature more rapidly than current resuscitation guidelines and surgical textbooks would indicate.
Admission hematocrit and transfusion requirements after trauma
2013, Journal of the American College of SurgeonsCitation Excerpt :Conventional teaching is that Hct remains stable for several hours after trauma because “patients bleed whole blood” and compensatory mechanisms for fluid shifts have not taken place.1-3 There are multiple controlled studies in animals18-22 and humans23-26 showing an almost immediate drop in Hct after hemorrhage with or without fluid resuscitation. In a pressure-driven model of severe hemorrhage in dogs, Prist and colleagues22 analyzed the rate of transcapillary refill using dilution of tagged RBCs.
A new parameter for the diagnosis of hemorrhagic shock: Jugular index
2012, Journal of Critical CareCitation Excerpt :Similarly, in our trial, the blood pressure and the pulse rates varied after blood donation, but these values were within normal limits. Among trauma cases, the diagnostic value of hematocrit is also limited because of administration of intravenous fluid and erythrocyte suspension [13,14]. In a retrospective trial, the initial hematocrit values on admission, which were determined to define patients who required surgical intervention, were reported to have low sensitivity [13].
